Broadway Blvd. #207 Tucson, AZ 85710-3524 Student Merit Scholarship APPLICATION INSTRUCTIONS The Arizona AER Scholarship Project is sponsoring one $500 Merit Award recognizing an outstanding visually impaired student. Applicants must be currently attending a post-secondary educational program or be completing their last year of high school, along with having central acuity of 20/70 or less with best correction or a significant peripheral field loss of 20º or less. In addition to the Application Information Sheet, applicants must submit the following items: 1. Two letters of recommendation: one from a current AZ AER member who will act as your sponsor and one from an academic advisor, teacher or community leader 2. A typewritten essay of no more than two pages describing the applicant’s educational, professional and personal goals and how this award will help achieve these goals 3. A certified copy of the applicant’s student transcripts from the school or institution in which the student is currently enrolled. 4. A vitae or résumé of the applicant’s work experience, extra-curricular activities and honors. Selection will be based upon evaluation of submitted materials reviewed by a committee appointed by the Executive Board of Directors of AZ AER. Notification will be made to all applicants by July 1, 2008 and the scholarship awarded by November 1, 2008. Recipients will be recognized and honored at the Fall AZ-AER Annual Conference.
